Wes Streeting has entered a battle against a beast that cannot be tamed unless doctors rightfully get FPR. After telling patients last week they should always get to see a Doctor he will pay those with absolutely no medical training (ACPs) more than what doctors are asking.
This is why @wesstreeting wants to flood the NHS with medically unqualified, unregulated ACPs . To replace Drs on drs Rotas YES , I said unregulated, @nmcnews @The_HCPC etc don’t regulate ACPs practising medicine .
Imagine you go to medical school for 6 years, graduate with a distinction, do your foundation years, do 2 years of EM training, pass MRCEM exams, and find yourself disagreeing with a plan made by your ACP colleague. Sucks for you, tier 2 for you, tier 3 for thy.
Would love to understand from RCEM and Dorset how this works ‘Senior decision maker in all areas in ED’… as an ACP So is that a working on tier 3 or 4 of ‘medical’ rotas?
If someone replaces a pilot and hasnt done any pilot training, then states they’re actually doing the same thing as the pilot but offering a different perspective… Are you getting on that plane? Are you getting your family on that plane?

1/n I want to say something about PAs. I don’t know yet if this will be long or short cause I am not psychic. What I do know is this: the situation has become a mess. Not just administratively. Not just professionally. But emotionally. And it’s going to leave scars.

We literally know more about medicine than doctors. We can easily manage long term conditions. We’ve been working in GP surgeries for last 20 years alongside our GP colleagues. Most would beg to differ.
As a doctor who’s passed exams in haematology, I would NEVER take this post! Yet an ACP, with NO medical degree & NO requirement of experience in haem will be placed on haematology REGISTRAR ROTA as senior decision makers?!! Haem pts are + complex & sick! This is criminal! 🚩
To say that I am shocked about this job advert is is an understatement @nottmhospitals I hope you realise how complex and vulnerable Haem patients are, and how utterly dangerous this is, letting medically unqualified people play registrar in an extremely complex speciality 1/
“Working across role boundaries.” These boundaries exist for a reason. It’s to protect patients from harm. The job description below is that of a senior paediatric dr. Complimentary roles are welcome. Substitution is not acceptable. Why not fight for more paeds trainees?
‘We are the Royal College of Emergency Medicine, not the Royal College of Emergency Physicians’. Take the hint, EM Doctors. They are coming for Tier 5 next. UK EM will become a protocolised triage service, & no more. Leave the specialty or leave the country.

How long has it been the case that someone who isn’t even a doctor can work on a doctor’s registrar rota? And very importantly for patients, what standard would someone in this kind of role be held to in a negligence claim? That of an advanced practitioner, or a doctor?

So if it is not deemed safe for PAs to request ionising radiation, particularly in the context of undifferentiated patients why did ACPs get a pass all those year ago. Surely it is time to revist that decision?

Do you not think members and fellows should be celebrated separately to reflect their unique challenges and hardships in reaching said stages? What is the need for ACPs when the specialty is so over-subscribed?
Well done to those who obtained their MRCEM and FRCEM. I'm sorry your royal college made you share the moment with those who haven't remotely a fifth of the training you have but will be so much more employable than you. RCEM sold out doctors and patients years ago. SHAME.

Not a bad idea. But healthcare staffing should treated as equals. This should also apply to Nurses, Physios, OTs etc
