I often get asked if admitting to ICU for early pressors will clog up beds. My response: 💡 Admit early, stabilise with pressors, and they’re out in 24h once the antibiotics kick in. OR 💣 Wait two days, drown them in fluid, and admit them with multi-organ failure. Now they’re in…
💉 “Give a fluid challenge — see if we can get the norad down.” We’ve all heard it. Boluses are often given just to reduce vasopressor doses. Why? • Fear that pressors are harmful • Pressure to keep patients off ICU But topping up to drop the norad might not be the best move…
Thank you @jeremycorbyn for visiting our regional WM @BMAResidents doctors picket line. Where is @wesstreeting? No doctor wants to strike. We want to be at work doing what we spent years training for. Today, we stand together @TheBMA because there's no other choice.
Todays picket at the Queen Elizabeth Hospital in Birmingham🪧 £22.67/hr is not an unreasonable ask. Doctors have subsidised the NHS for far too long. Perhaps @wesstreeting needs reminding that the power to stop these strikes lies squarely with the government. @BMAResidents

Japanese design firm Kenji Abe has designed a glass beer bottle that you can stick into the sand on the beach
Yesterday’s Ed & Development Day focused on pre-hospital maternity & neonates 👶 We ran 3 sims + received expert talks from Dr Caroline Leech & Camella Main, and brilliant neonatal resus masterclasses by Andrea & Anneke 🙌 Big thanks to Dr Rachel Edwards for leading the day!
Maternal cardiac arrest in-hospital is not the same as out-of-hospital 🚑 In-hospital survival - 54% Out-of-hospital - 5% Important learning points from @LeechCaroline at the @wmcareteam education & development day 🚑

D.I.Y. umbilical cord stumps for UVC practice 🤩 Cost me about £20 in total to make 10 stumps.

@RCollEM if you even consider moving to credentialing PAs I will resign my College Tutor role and resign my membership
6/ “Credentialling” creates a separate track: from novice to “advanced” assistant, with escalating clinical rights — but no requirement for medical school, postgrad exams, or specialist training. It’s a parallel system.
Interesting learning from Dr Patrick Davies…. There is such a thing as reheating too quickly in hypothermic cardiac arrest 🥶 Rapid rewarming can lead to neuro inflammation & metabolic disturbances. #NEPTUNE25

📢 New National Pre-Alert Guideline RCEM and @AACE_org have released a new national guideline to standardise pre-alert calls between ambulance services and EDs. 📄 Full guideline available now: tinyurl.com/rcemAACE ✅ Updated criteria for pre-alerts ✅ Use of ATMIST…
Absolutely brilliant talk from Dr Jake Turner on paeds traumatic cardiac arrest. Highlighting the importance of training your team on how to deliver rapid warmed blood product transfusion to small patients - Belmont not ideal 🩸 #NEPTUNE25

Are you involved in, or considering a career in prehospital care? EPiCC, a new national survey, looks to understand EDI within prehospital critical care, to understand how this could be changed. Click the link and help inform the future of PHEM redcap.aakss.org.uk/surveys/?s=LCH…
We’ll be closing this for responses tomorrow! A huge thanks to everyone who has responded so far.
Myself & @ThomasBurton12 are carrying out research into the short & long term psychological impact on police officers of witnessing prehospital resuscitative thoracotomy. Please re-tweet, share & send to anyone you think it might be relevant to.
