As someone who does A LOT of neuro and spine cases, I 💯 agree with everything on this post. While there have been breakthroughs especially with minimally invasive spine surgery, I would consider non-operative options first and then surgical intervention.

Unless absolutely necessary, I would advise against spine surgery. We see many failed backs. There is indeed a role for spine surgery, as I can personally attest to, but be sure to exhaust all non-operative options first. Once a person has surgery, they're never the same!
As Long COVID funding gets cut by government and private funding (due to unaccounted for taxpayer money spent), the next big money train are the COVID vaccine injured patients.

I’d add MDL for Lyme as they do a v good western blot and generally take insurance. Igenex etc also v good and can test for panel of diseases. For just bartonella, I like Galaxy Diagnostics @Bartonella.
💯 If you are doing clinical trials on long COVID (and on post vaccine) and you do not screen and exclude Lyme your trial will fail. Simply asking ‘did you have Lyme disease’ on a study enrollment questionnaire is not a screen for Lyme disease. Blood work is the key! Many…

Any plans for a case series for these 30 patients? If I can work 60+ hours a week in the OR and write up an 18 patient maraviroc case series on my free time (and without any compensation), I am sure others can crank out some case series publications.
I am unclear as to the mystique surrounding Pemgarda as this beacon of unattainability. Approval is straightforward - I’ve done it for 30+ patients, ~10 more pending. It’s the eye of the tiger. Private practice equates to freedom to be proactive and do what’s best for patients.

This is a very interesting topic and we are pulling together some data on this right now. The general gist of our data, though: just like COVID can reactivate latent herpesviruses, I think we have emerging evidence to suggest it can also reactivate latent tick-borne infection 😐

Those CD16+ monocytes are like garbage trucks on fire filled with crap (viral and bacterial debris, dead cells, etc). Trials and studies validate certain biomarkers and test drug efficacy.
That is exactly what we are doing and have a chronic Lyme trial planned. We also found Borrelia protein fragments in monocytes #lyme
