We should talk about the the inevitable q of how this mess happened. Prof Leng stated her review wouldn’t dwell on it. This is within her gift - the review is hard enough to land despite sticking to its precise question. But the question of how this happened cannot be ignored 1/

Competitive entry, difficult and long course, student loan debt, randomly allocated across the four nations, used as cannon fodder for two years, paid less (relatively) than what they used to be, specialty training unreachable, unemployment looming. Any surprises?

‘We are the Royal College of Emergency Medicine, not the Royal College of Emergency Physicians’. Take the hint, EM Doctors. They are coming for Tier 5 next. UK EM will become a protocolised triage service, & no more. Leave the specialty or leave the country.

I look forward to RCEM Tier 6 - can be College President; award increasingly meaningless participation diplomas to all & sundry; may or may not need any healthcare qualification to apply.

It also confirmed hospitals have used them as substitutes for drs...no surprise there. Hospitals have been doing same with nursing associates for years, as with many other lesser trained roles. This is taskification of care, now Govt policy in NHS 10 year plan #patientsafety

So a higher standard of medically-led care delivers better patient outcomes, cheaper. Take note @wesstreeting & the wider NHS.
