The #LengReview recommends that #PhysicianAssociates should work within a tightly-defined scope of practice in GP - not seeing undifferentiated patients, instead providing health checks & lifestyle support. But IMO the maths doesn't work - why pay Band 7 salary for a Band 3 role?
Myth: #delirium isn't a priority for detection Fact: delirium linked with ~20% mortality at 30 days in older people attending hospital.
RFK Jr. went on national TV and spouted egregious, dangerous falsehoods about vaccines. As a parent and infectious diseases doctor, I couldn't stay silent. @FoxNews might not fact-check him, but I will. I've reviewed the trials. I've catalogued them. I have receipts. 🧵
RFK Jr is on Fox News going full anti-vax
I have met all of them, worked with some of them And i doubt any are under 65, with most well into their 70s So what they think about industrial action or contracts for doctors in their 20s and 30s is irrelevant their time has now gone and it is not their future
You’ll begin to persuade me that AI can attempt complex decision making when I’m not held at a red traffic light on empty roads when nothing is coming the other way
#EHJCVI 🔎 What’s the link between ECV, scar burden, myocyte volume, and diastolic dysfunction in moderate AR? #whyCMR shows these parameters increase with worsening diastolic dysfunction 📉Is there an association with mortality? Read more 👉 doi.org/10.1093/ehjci/…
There’s an effort to frame this as a basic industrial dispute, when it goes much beyond that. People have asked questions re why the PA situation has inflamed doctors and why it spills into the pay dispute. I want to start with explaining PAs compared to doctors’ training: 1/
I took 24 hourly snapshots of how many patients were in A&E for each of the 366 days. Then I calculated the average number of people occupying space in A&E per day and coloured the barcodes according to whether the day was more crowded (blue) or less crowded (red). 10/12
We can take this barcode of lengths of stay and colour-code it. Let’s make the 183 days with the most attendances red, and let’s make the 183 days with the fewest attendances blue to see if it helps us make sense of things. 6/12
Why does X keep putting these loud mouths on my feed. Spending more time blocking/ muting rude ignoramuses than actually getting anything worthwhile from this platform at the moment.
So the old gent over the road doesn't get his knee done and remains in pain because wankers like this man demand money with menaces. Absolute bunch of greedy entitled selfish shits.
It’s slightly painful how stupid this is.
Hospitals administered the “death protocol” of remdesivir and ventilators. You had a higher risk of dying “from Covid” if you went to the hospital.
Soon after describing this finding, however, Trousseau himself developed thrombophlebitis & told a confidant, “I am lost; a phlegmasia which showed itself…leaves me no doubt about the nature of my affliction." His story is described beautifully @NEJM: pubmed.ncbi.nlm.nih.gov/12374880/ 9/
I still am yet to hear a convincing argument as to how the GMC can reasonably justify the amount doctors pay them. They continue to publically renounce functions one would expect of a regulator. Great salaries, pensions and benefits… justified by what contribution, exactly?
One of the most important criticisms of the now infamous paper and more convincing evidence that the conclusion that “medical knowledge is comparable between cohorts” is the opposite of the truth 👏
Here's something else interesting about the PA knowledge comparison study: If we plot performance by question, medical students & FY1s show nearly identical patterns (r=0.927), but PAs show virtually no correlation with either group (r=0.045, 0.008).
IMT applicants should NOT have to sit the MSRA. Making people sit exams that are tenuously relevant to their specialty in order to make selection easier is just another way to fuck over incoming IMT cohorts. This should be an easy, hard no from RCP.
The GMC has been 100% politically captured, it may as well move to Tufton Street, it started when the Director General of Acute Care & Workforce was appointed from DHSC to run the GMC. I have no confidence in the GMC or Mr Massey, root & branch reform is needed and now.
The GMC are endorsing UMAPs supervision document of autonomous practice for undifferentiated patients This puts them in direct conflict with the @TheBMA, @rcgp, @RCPhysicians & the health secretary The regulator has gone rogue What are you going to do @wesstreeting?
🚨 BREAKING: Resident doctors in England have re-entered a pay dispute with the Government. Why? Because progress is being stonewalled by the continued delay in the publication of the DDRB's pay recommendations. Trust must be rebuilt. Publish the DDRB now bma.org.uk/residentspay
