DOAC related ICH is a devastating condition with 40% rate of hematoma expansion and nearly 30% mortality. In addition to usual ICH management, should we routinely consider using Andexanet Alfa for DOAC reversal in ICH patients? Below is a thread 🧵
My analysis of patient-reported outcomes in the AQUILA trial (dara v active surveillance HR SMM). I graphed out the PRO scores in the @NEJM supplement in this thread; follow along! 🧵#mmsm #ASH24 nejm.org/doi/full/10.10…
Well, well, this is quite a story that I had not known about. Re: PLATO trial and ticagrelor bmj.com/content/387/bm… Thoughts welcome.
#ASH24 #mmsm Few important observations on AQUILA This is in top of prior observations I shared 🧵
FDA's own review of thrombotic events reported 12 extra cases – 9 in andexanet & 3 in usual care – that were not assigned as thrombotic by blinded committee. Sorry, I don’t have more info on this! Final figures (based on n=471 FDA safety set): 14.6% vs 6.9%, stroke 5% vs 0.4%.
Working on an anemia lecture this morning, and reviewing the cbcmadeeasy.com website. Fantastic work by some of my fave people. (And so far, I'm doing pretty well on the quizzes...) Check out this interactive, fun, and challenging resource! @stefan_d_jevtic @SMithoowani

Most patients with sepsis don't need anti-anaerobic antibiotics. But most get them anyway. Why not, what's the harm? I worry the harm is considerable. Our new study in @JAMAInternalMed: using a 15-month pip-tazo shortage to answer this question. 1/n jamanetwork.com/journals/jamai…
Guidelines are unanimous; For look risk procedures like paracentesis we need not check INR and should not give FFP Liver, gastroenterology, hematology, and interventional radiology societies agree So why the variation in managing “the cirrhotic coagulopathy”? 🧵
Before diagnosing an elderly person with confusion secondary to a “UTI” you should have to list 5 other likely differential diagnoses and explain why you think they are not the cause
This a tweet to bring attention the need for delayed release of records after provider review that foolishly was not thought of by Congress when they passed the open notes act and nothing more.
A previously healthy woman presents with weakness & unsteady gait six weeks after surgery. On exam she has a broad based gait & decreased sensation in her lower legs. Blood smear reveals the following. What anesthesia agent did she receive & how does it explain her symptoms?
Wow looks like Shoppers Drug Mart is now forcing you to see the pharmacist for a consult for previously shelved items (presumably so that they can bill OHIP). 20 people in line with 1-2 hour wait on Christmas Eve just to get an OTC med. Pharmacist friends, am I missing something?
Shoppers has been exploiting their employee pharmacists for years, a nice example would be pushing quotas for completing meds checks at $60 a piece. You’re better off going to a privately owned pharmacy!
