EM/ICM/PHEM doc. LOVE learning + teaching. I work in the sky above Sydney, have the attention span of a bullet, + often have days that are like cartoons #FOAMed
I showed a resident how to diagnose a pneumothoraces She asked 'don't you mean a pneumothorax?' I replied 'it's pleural'
(5/x) The most common error I see in phenotyping septic patients is assuming that they are having adequate cardiac output, assuming that they have hyperdynamic circulatory function, and assuming that they are fluid responsive. For many patients, this does not hold true. Early…
Nice!
Lactate around 20 has everyone flapping but the patient is alert and well perfused Time to use system 2 thinking and understand the causes of elevated lactates This is NOT a patient dying of sepsis or organ hypoperfusion In this case it was an alcohol dependent patient whose…

Multimodal diuresis is the way to go for decongestion Another gem of a thread from @ross_prager
(0/x) Do you want to be the Obi-Wan of diuresis? (or Darth Vader if you tilt to darkness) Are you still starting diuresis with 40mg of 80mg of IV lasix? Below are the lessons I learned from @ArgaizR and @FH_Verbrugge on our most recent HCProunds as they show us how to wield…

Many see pre-procedural checklists solely as a tool for checking equipment In EMRS we monitored the heart rate of clinicians during our pre-anaesthetic checklist. 100% of clinicians experienced a fall in heart rate Here's why.... CoreCognition.co.uk
Think you’ve mastered septic shock? These patients are still one of the biggest challenges in the ED. Early norepi? Restricted fluids? Admission criteria? At #Incrementum2026: “Things You Shouldn’t Forget in Septic Shock” with @srrezaie & @SaraCrager.
CPR is life saving. But clinicians should understand just how abnormal this way of providing a circulation is, and how chest compressions can really smash the lungs! We still don’t know the best way to ventilate during CPR, but there’s no doubt that in many patients we are not…
❤️ 🙏🏻
This is the video I point my trainees to when they start charging around shouting sepsis for every raised lactate they see!! Best 13 minutes they will spend learning about lactate!!
We had a similar patient that was put on pressors for treating a number. It was a good way to teach the residents to ask pertinent questions of a woman who didn’t like the food at the nursing home and didn’t eat for almost 2 months
100% agree. Also, no need to blindly treat a mildly elevated lactate w IV fluids
Hats off to our amazing ED docs and nurses for a smooth intubation today of a potentially very challenging airway This is why I love working on the Northern Beaches of Sydney
Emergency medicine knows no borders. And education shouldn’t have them either. At #Incrementum2026, we believe high-level training should be accessible to everyone — that’s why we offer a range of scholarships. 🔗 Visit incrementum-conference.com and check them out.
