It almost sounds like Wes gets it. Silly incentives and targets are set when people who fundamentally don’t understand EM (and don’t have the curiosity or humility to speak to those that do) try to ‘fix’ EM. Here’s hoping this marks a change in focus. Time will tell.

But he leaps to the assumption that the solution to long waits (in major A&Es) is keeping people out of hospital. This idea is bad: It isn't the problem in major A&Es and decades of experimentation have shown the NHS doesn't know how to achieve it anyway...

The major cause of ER overcrowding is the lack of fully funded & staffed in-patient beds. It always has been. EDs aren't designed to hold patients long-term. It's also less safe for the admitted pt. Walk-ins are easy. It is time to actually spend money on admitted pt areas.
