This is completely contrary to advice we get regularly to confront colleagues where they are engaging in behaviour against our core standards (if safe to do so). I don’t work for the NHS but if the culture is to never confront a colleague about conduct and instead tell a manager…

This is *extremely* problematic HR practice. Inviting someone to review & agree notes is normal, making small corrections from their feedback is normal. Doing this in a secret 1:1 conversation with no record of changes made, and destroying the original recording, is NOT normal.
HR let Dr Upton edit the notes of his interview - notes that were produced from the recording of the interview (the recording is destroyed)😱😱😱 They had a Teams meeting to edit the original notes of his interview (no minutes were taken) #SandiePeggie
