The way to embed continuity of care is to have the right workforce and systems. Healthcare has lost a huge amount of care managing (not only care delivering) professionals, its also abandoned working practices like primary nursing SMS might detect it but it wont deal with it.
Leaders should consider adopting a safety management system, embedding continuity of care, and restorative practice
