Clue to spinocerebellar ataxia 27B (SCA27B): may have high T2/DWI signal in the superior cerebellar peduncles and its decussation. Seen in around half of patients! Reading: - neurology.org/doi/10.1212/NX… - link.springer.com/article/10.100…

Always a great talk from @NeuroradLal on SIH #Radiopaedia25! Pearls: - MRI brain: new to me is collapse of optic sheaths - MRI spine: main role is SLEC (dural defect) vs no SLEC (CSF-venous fistula), fat-suppressed is best, go down to the sacrum, flow void sign can help localise

Ischaemic stroke due to acute basilar occlusion can rarely present with seizure or convulsive movements as part of the initial/early clinical features. Something I have only seen once - worth knowing this is possible! pmc.ncbi.nlm.nih.gov/articles/PMC87…
Important from @francisdeng #Radiopaedia2025 re: neurovascular compression of CN VII - from the root exit point to the proximal cisternal segment. Don't forget that the initial segment of CN VII is the 'attached segment' which lies against the pons, all centrally myelinated.

Helpful pearls from @tmcampion on my catch-up of Day 1 #Radiopaedia2025 regarding suspicious imaging features of intravascular lymphoma. A diagnosis we think about a lot, but diagnose very little of!

Great diagram, from an interesting review, summarising variants of MND, there are just so many beyond ALS/PLS/PMA/PBP! Source: pubmed.ncbi.nlm.nih.gov/30846210/

Clinical pearl: when measuring orthostatic hypotension also measure the change in HR. A ΔHR/ΔSBP ratio lower than 0.5 bpm/mmHg is diagnostic of neurogenic orthostatic hypotension, e.g. in MSA. pmc.ncbi.nlm.nih.gov/articles/PMC58…
This is a really cool potentially new treatment for Parkinson disease: intracerebroventricular administration of anaerobic dopamine via an abdominal pump connected to a subcutaneous catheter implanted in the third ventricle (near the striatum). pmc.ncbi.nlm.nih.gov/articles/PMC11…
