The Gleason score - the fundamental determinant of how we chose on treatments in localized prostate cancers - basically doesn't make any sense.

I keep hearing "AI models have to be explainable otherwise doctors and patients won't trust them". But if I Google "does explainability improve trust in AI?", I get an AI summary (short answer "yes") created by an AI process that probably <1% of the population understands.
Results of observational study almost perfectly match those of our RCT. Hazard ratios of 1.07 / 0.72 for BCR / mets; RCT reported 1.05 / 0.82. Conclusions: do extended PLND during radical prostatectomy; effects of PLND occur after BCR.
Confirmation of the results of our RCT showing that PLND reduces metastasis without affecting BCR. @UrologyMSK @Uroweb @EUplatinum @JUrology @VickersBiostats #prostatecancer

Note how "IV magnesium associated with less AKI" in the paper becomes "IV magnesium reduced AKI risk" in the tweet.
Simple, but yet effective. Giving IV magnesium before cisplatin reduced AKI risk! A small step that protects kidneys while treating cancer. #Oncology #Nephrotoxicity #cisplatin #OncoNephrology @JAMAOnc @OncoAlert @OncBrothers
"Patients with grade group (GG) 1 on biopsy can have higher grade disease on surgery. So GG1 = cancer". But also true of benign: "Patients with benign on biopsy can have higher grade disease on surgery"
Sounds great. It’s good that you’ve never over promised before and the American public can trust you.

🚨New in @EUPlatinum! 📌 PSMA PET for Prostate Cancer Diagnosis & Staging by @elio_mazzone et al 🧠 This comprehensive meta-analysis confirms the high diagnostic value of PSMA PET—especially when combined with MRI—but reminds us it’s not enough to skip biopsy or lymph node…
🧠 Words matter in science. 📢 In their @EUPlatinum article, Vickers & Inman call for stricter labeling of clinical trials: ❌ Don't call single-arm studies “phase 3” ❌ Don’t mix “registrational” with “phase 2” ✅ Let’s keep trial terminology precise—for better science &…
Systematic review: "PSMA PET cannot be used alone to determine the need for lymph node dissection (LND)." sciencedirect.com/science/articl… PSMA and anti-LND advocates: "let's use PSMA to stop doing LND"

Psychometricians develop questionnaires, then require permission to use / modify. Is there any other scientific methodology, whether lab (eg PCR), statistical (eg ttest), design (eg factorial), psychologic (eg cognitive debrief), where originators demand such permission?
Let’s just all agree to accurately quote him.
This tweet, like many others you've posted in recent days about Vinay, and about the new FDA that could be worse than the old one etc., are, in my view, in very poor taste, for several reasons. 1- "VP", as you call him, has shaped many of your ideas over the years 2- Vinay has…
Forgive me for not trusting your "common sense" @DrMakaryFDA Let me know when you and @VPrasadMDMPH have any RCTs to show us.
