There are serious problems with peer review. It kind of reminds me of Winston Churchill's famous quote. It is the worst form of quality control for the medical literature, except all the other that have been tried.

Randomized trials in individual patients – N of 1 #RCTs - remain underutilized. This @AnnalsofIM paper describes our initial experience with 57 N of 1 RCTs. In 15, results prompted patients and clinicians to alter their treatment plans. pubmed.ncbi.nlm.nih.gov/2297206/
