A painful ECG for the STE millimeter-criteria purists! It’s hard to find a single feature on this tracing that isn’t clearly diagnostic of anterior STEMI yet the EKG does not meet formal millimeter criteria. Diagnosis: mLAD occlusion.

Great engagement and answers so far! Two quick comments: 1. The automated diagnostic statement is not AI, it's an old rule-based algorithm from the 80s (see link in bio to reveal AI ECG interpretation). 2. Is the algorithm wrong? Does this ECG meet ST-elevation criteria?

Answer: Hyper-acute Anterior STEMI due to (re)-occlusion of the LAD. Fortunately this patient was correctly identified based on the subsequent ECG and promptly managed!
