🔱The reversed trident sign, with ventral spinal cord enhancement, mirrors the classic trident sign seen in neurosarcoidosis but with an atypical distribution. Unlike MS or NMOSD, which lack this pattern, it pointed toward spinal cord sarcoidosis. doi.org/10.1212/WNL.00…

Also,🚨 kidney dysfunction (eGFR <45 mL/min/1.73 m2) was associated with higher p-tau217 (i.e., false positive results). To ensure accurate interpretation, we recommend measuring creatinine (and eGFR) together with p-tau217.

Stridor is a common and presenting feature of IgLON5-antibody disease. This Research Letter describes an additional inspiratory noise distinct from stridor, in three patients, observed independently across two centers. ja.ma/3HkZD1y

Beware commercial tissue based assays (TBAs) to detect cell surface autoantibodies in neurology. TBAs are often used as screening tests and are reader dependent. Bottom line: please think clinically when diagnosing autoimmune neurological syndromes pubmed.ncbi.nlm.nih.gov/40393020/
