I hear interns and residents banally say they had to prioritise due to time limitations so they couldn't do a full note. I see the use of copy-paste. I feel clinical reasoning skills are changing (to put it politely). I fear what might be next. #MedEd needs to adapt at all levels

Beyond the long-term educational issues, this also really validates the concerns from our NEJM editorial @AdamRodmanMD @arjunmanrai. Divorcing reasoning from documentation changes the underlying cognitive process, and we can't assume without evidence that they are equivalent.
