Agree with aortobifemoral bypass but mesenteric have to be revascularized either staged or during , if not you are asking for trouble particularly during perioperative period …
Assuming the patient is fit for open surgery. Could get away with an aortobifemoral bypass sewn infrarenal but with suprarenal clamping. Depending on how bad the mesenteric symptoms are, probably bypass off the graft to the SMA.

Agree Joe…identifying features of unstable dissections and using therapy to prevent the long term sequelae of TBAD is an absolute needed answer … we currently have a decent roadmap to better trials
The literature is scarce on my fronts with TBAD. But making an attempt to delineate prognostic features should be a main driver for future investigations. High risk features are an important first step in this process. IMO, The “crap” is usually personal bias …
